Try Imaging4Notes

My company has been using these guys for about six months now:

We use their software to scan a piece of paper, turn the result into a PDF, then place the PDF in a RTF. It took a lot to get it up and running, but has been problem free since then.

They have an OCR module. You'll need that, but since we don't, I can't tell you if it will do what you need.

The main thing we liked about this software was that it didn't require a new server. That's a really big deal for us.
